DPR-50 The subject report is forwarded for Headquarters action as follows. 1. Inclusion of Regulatory Guide 1.71 in Licensee's Q/A Program The licensee currently addresses the conditions of limited access- ibility identified in Regulatory Guide 1.71 as indicated in Detail 4; however, since the licensee has no requirement to conduct this mock-up training when these conditions are encountered in the field, the current practice could be terminated at the licensee's 9 discretion. Section IX of the licensee's Quality Assurance Program contains commitments to Regulatory Guides 1.37 and 1.39 as well as e ANSI N45.2.1 and N45.2.3. It is recommended that the licensee's 3 ' program be updated by effecting the necessary licensee actions to include Regulatory Guide 1.71 in his Quality Assurance Program. 2. Revision of FSAR Analysis with respect to NaOH Tank Loading As noted in report Detail 7b, there are major discrepancies between the safety analysis presented in Supplement 1 to the FSAR and the values required for Sodium Hydroxide Tank loading in Section 3.3.1.3.b of the FSAR. The licensee indicated that a new analysis had been presented to Licensing by the NSSS supplier, but not yet incor- pcrated into the FSAR. It is requested that the inclusion of the new analysis, if appropriate, be expedited by Headquarters since Licensing has been aware of the appa*:ent inconsistencies documented in Detail 7b, both from licensee and RO:I inputs. .- u /. sb.u.,-w /, ' Csy r w w, Eldon J. Our inspector also verified the steps you had taken to correct the violation brought to your attention in a letter dated October 4,1974, following our inspection number 50-289/74-20. We have no further questions regarding this matter. During this inspection, it was found that certain of your activities appeared to be in violation of AEC requirements. The items and references to the pertinent requirements are listed in the 2nclosure to this letter. This letter constitutes a notice sent to you pursuant to the provisions of Section 2.201 of the AEC's " Rules of Practice", Part 2, Title 10, Code of Federal Regulations. Section 2.201 requires you to submit to this office within 20 days of your receipt of this notice, a written statement of explanation in reply, including: (1) corrective steps which have been or will be taken by you, and the results achieved; (2) corrective steps which will be taken to avoid further violations; and (3) the date when full compliance will be achieved. DESCRIPTION OF VIOLATION . Metropolitan Edison Company Docket No. 50-289 License No. DPR-50 Certain activities under your license appear to be in violation of AEC requirements. These violations are considered to be of Category l II and Category III severity. Category II Violations 1. 10 CFR 19.12 states in part: "All individuals working in or frequenting any portion of a restricted area. . .shall be instructed of their respon- sibility to report promptly to tne licensee any condition which may lead to or cause a violation of Commission regulations and licenses or un- necessary exposure to radiation or radioactive material; ...and shall ' be advised as to the radiation exposure reports which workers may request 9 pursuant to 19.13." p In violation of the above, all individuals working in or frequenting d restricted areas were not instructed in their responsibilities to repert conditions which could cause or lead to violations of Commission regula- tions; in that personnel questioned stated that such instruction has net been given, and that there was no documented training for such instruction. 2. Technical Specification 3.3.1.3.b states in part: "The sodium hydroxide tank shall contain not less than 16,000 pounds of sodium hydroxide and not more than 17,000 pounds of sodium hydroxide." - In violation of the above, the Sodium Hydroxide Tank was found to contain 17,677 pounds of sodium hydroxide during the routine surveillance test conducted on Septe=ber 19, 1974. (A0 74-17, Le .:er to RO:I dated September 30, 1974) We note that item 2 wu reported to Regulatory as indicated above. Your corrective measures wure reviewed during this inspection. Criterion V of Appendix B to 10 CFR 30 and Section V of the licensee's Operational Quality Assurance Program both state in part: " Instructions, procedures, or drawings shall include e.ppropriate quantitative or quali- . tative acceptance criteria for determining that important activities have - been satisfactorily accomplished." In violation of the above, Emergency Procedure 1202 . MSS OF RC/RCS . PRESSURE had inappropriate weeptance criteria for determining when sodium hydroxide addition had been satisfactorily accomplished; in that step 8 A (3) of the r ocedure indicated addition was complete and certain valves were to be repositioned when the sodium hydroxide tank reached . " low level." " Low Level" was not quantitatively or qualitatively defined in the procedure nor indicated on the level instrumentation.
