CO:;CRETZ VOIDS AT TZ' DON GALCRY Division of Compliance Inquiry Menorandum 289/68-A of December 17, 1968, mentions concrete voids near three tendons in the tenden gallery of the containment building, apparently caused by scrap vood and mud bein7 present in the forms at the time of concrete place =ent.
It appears that, taken in therselves, the observed voids.uith the dimensions given, can readily be repaired by some means such as the proposed epoxy cement grout witnout any question arising ns to the integrity of the in-place mat concrete.
However, these voids may indicate that the construction procedures or degree of inspection for the mat may have been inadequate. It would be desirable, therefore, to obtain the followin; informaticn:
l.
Details of pour procedure (number and sequence of pours, height of individual pours, etc.).
2.
Description of inspections carried out prior to, and at time of, pour, and the quality and depth of these inspections.
3.
Infor=ation as to the overall quality of cleanup and form placement.
4.
An outline of the repair program, when finalized.
