License No. DPR-50 APPENDIX A NOTICE OF VIOLATION Based on the results of an NRC inspection conducted on November 7-10, 1977, it appears that certain of your activities were not conducted in full compliance with condit ans of your NRC Facility License No. DPR-50 as indicated below. These items are deficiencies.
i A.
10 CFR 50, Appendix B, Criterion VI, states in part, " Measures shall be established to control the issuance of documents, such as... drawings, including changes thereto, which prescribe all activities affecting quality. These measures shall assure that documents including changes...are distributed to and used at the location where the prescribed activity is performed." The Approved Quality Assurance Plan, Rev. 6, Section 22 states, in part, "The Manager-Generation Engineering is responsible for providing and implementing procedures for the preparation and maintenance of design records such as...as-built drawings."

On November 9, 10, 1977, Drawings 201-091, 210-109, 307-079 ShF in the apertive card files did not agree with the revision number contained on the latest index of as-built drawings l
dated August 31, 1977.
On November 9,1977, Drawings 153872, Rev. 4 (Mod 903) and 36416000, Rev. 3 (Mod 814) at the site were not marked to reflect a revision was in progress as required by AP1001.
On November 9, 1977, Drawings and/or aperture cards of the 400-XXX series were not present at the site as required by GP 1015 (Administrative Controls for Preparation, Approval, and Revision of TMI Drawings to show As-Built Conditions).

such as duration, location, and assigned responsibility." The Approved Quality Assurance Plan Rev. 6, states, in part, " Record storage facilities are in accordance with the applicable require-ments of ANSI N45.2.9 (Draft - January 1973)."
Contrary to the above.
On November 9,1977, the Control Room Log Book, Shift Foreman's Log, Operating Logs, Computer printouts and Operating Parameter Charts were not being maintained in an area that satisfies the requirements of ANSI N45.2.9 nor is duplicate storage provided.
On November 10, 1977, the Change / Modification Checklists were not being maintained in an area that satisfies the requirements of ANSI N45.2.9 nor is duplicate storage provided.
