June 29, 1979 PRELIMINARY NOTIFICATION OF EVENT OR UNUSUAL OCCURRENCE--PNO-79-204 Tnis preliminary notification constitutes EARLY notice of events of POSSIBLE safety or public interest significance.
The information presented is as initially received without verification or evaluation and is basically all that is known by IE staff on this date.

Subject:
IMPROPER INSTALLATION OF CONCRETE ANCHOR BOLTS During a reinspection of installed concrete anchors on 6/27/79, the licensee discovered that six anchor bolts had been cut short and tack welded to the support plates, giving the appearance of proper installation (i.e., that the remainder of the bolt was imbedded in the concrete behind the plate).
The bolts provide anchorage for safety-related control room ventilation ducting.
The licensee has issued a stop-work order to the contractor involved, secured all related records, and initiated a full investigation.
NRC RV inspectors, who are onsite investigating another matter, will audit and evaluate the adequacy of the action being taken by the licensee in response to the problem.
The licensee intends to issue a press release on the subject today.
Region V (San Francisco) received notification of this occurrence from the licensee at 8:30 a.m. on 6/28/79.
