PRELIMINARY NOTIFICATION June 22, 1979 PRELIMINARY NOTIFICATION OF EVENT OR UNUSUAL OCCURRENCE--PNO-79-178 This preliminary notification constitutes EARLY notice of event of POSSIBLE safety or public interest significance. The information presented is as initially receigd without verification or evaluation and is basically all that is knowu by IE staff on this date.

SAFETY RELATED CONCRETE FORM FAILURE (RADWASTE BUILDING)
On June 20, 1979, Regi n III was contacted by the Press regarding a structural failure at the Perry e struction site. During a subsequent followup inquiry regarding this matter :
Region III, a representative of the licensee's manage-ment reported that at 10: 15 a.m.,
on June 19, 1979, a 20' x 20' x 2' thick foot section of concrete forming failed during finishing of freshly placed concrete.
The concrete forming was a part of the rad waste building roof.
There were no injuries. The reason for the failure has not been determined.
The licensee is evaluating the cause.
Region III has informed OSHA of this matter and plans to followup as necessary.
The State of Ohio will be notified.
The local press is aware of the failure.
Region III Chicago received notification of this matter at 3:00 p.m., June 20, 1979.
