Subject:
FATAL ACCIDENT ON SITE The licensee reported that an employee at the batch plant had been found crushed to deatn in the compactor portion of the ice crusher.
The body was found at 12:25 p.m.
The man who was the Lead Operating Engineer had been working on a mechanical problem with the ice crusher during the noon shutdown.
This Preliminary Notification is issued for information only.
Media interest is expected because of interest in nuclear plants.
The State

of New Jersey and OSHA have been informed. A press release is planned by the licensee.
A press release is not planned by NRC.
