PRELIMINARY NOTIFICATION OF EVENT OR UNUSUAL OCCURRENCE--PNO-79-223 l

This preliminary notification constitutes EARLY notice of events of POSSIBLE safety or public interest significance.

The information presented is as initially received without verification or evaluation and is basically all that is known by IE staff on this date.

Subject:

XE-133 RELEASE TO THE PUBLIC AND UNAUTHORIZED BURIAL OF IODINE 125 The licensee reported two events:

(1) a release to the unrestricted area of Xenon-133 in excess of regulatory limits and (2) the inadvertent, unauthorized burial of Iodine-125.

On April 10, 1979, a chamber holding 4.6 millicuries of Xenon 133 was vented improperly to the environ.s. The average concentration released was 37 to 113 times the MPC for an unrestricted area, over a 40 minute period.

No member of the public was in the area at the time.

On May 25,1979, 5 to 10 millicuries of Iodine-125 were disposed of in a properly labeled radioactive waste drum.

The radwaste drum was emptied into normal trash, compacted, and trucked to a weste site in Wayne County.

The waste was disposed of in trenches 18 to 20 feet deep. The licensee surveyed the truck and the drivers and no contamination was found.

The custodian was surveyed thoroughly and his thyroid counted.

No activity was detected.

The State of Michigan and the NRC, Region III, Chicago, were notified by letter dated June 25, 1979.

RIII received the letter on July 5, 1979.

EPA Region V was notified by telephone on July 6, 1979.

Region III plans to conduct a special inspection.

No press release is planned by NRC:

however, media inquiries will be responded to if they occur.
