PRELIMINARY NOTIFICATION N or Jrn July 13, 1979 PP.ELIMINARY NOTIFICATION 0F EVENT OR UNUSUAL OCCURRENCE--PHO-79-241 This oreliminary notification constitutes EARLY notice of events of POSSIBLE safety or public interest sianificance.

The information presented is as initially received without verification or evaluation and is basically all that is known by IE staf f on this date.

Subject:

ALLEGATION OF IMPROPER CONCRETE PLA"EMENT A former employee of Guy F. Atkinson Ccmpany alleged to the Aberdeen Daily World newspaper, Aberdeen, Washington, that a concrete vibrator and approximately 8 feet of 1-inch air hose had become stuck and was left between the reinforcing steel during the placement of concrete in November 1978.

The concrete was being placed for a 10-foot thick shield wall foundation being constructed adjacent to the reactor containment building.

Members of the press, licensee representatives and contractor representatives were shown the section of foundation in question by the former employee.

The licensee has prepared a nonconformance report requiring the analysis of the effect on the structural integrity of the foundation, assuming the vibrator and attached section of hose were left in the foundation as alleged.

The licensee does not consider this matter to be reportable to the Commission at this time.

The NRC Region V inspectors, who are onsite on' a routine inspection, will as part of our routine inspection program, audit and evaluate the adequacy of the action taken by the licensee in response to the problem.

A preliminary evaluation indicates that the presence of such a foreign object would probably have negligible effects on the integrity of the concrete.

The licensee intends to issue a press release on the subject on 7/11 or 7/12/79.
