LOST SHIPMENT OF LICENSED BYPRODUCT MATERIAL The licensee reported that a radioactive shipment, one of seven boxes, containing a 500 millicurie molybdenum-99 generator (calibrated for July 20, 1979) had not been received by Waterville Osteopathic Hospital, Waterville, Maine.
The licensee reportedly transferred seven boxes containing licensed byproduct material to TWA to be shipped to the consignee, U. S. Priority Transfer Corporation, Bostoit, Massachusetts.
The consignee received the ship. ment on July 21, 1979, at 4:09 p.m., signing for seven boxes but claiming to have received only six boxes.
The six boxes were shipped to their destina-tions.
The licensee learned of the lost generator from a doctor at the Waterville Osteopathic Hospital during the morning of July 23, 1979.
Ine consignee has placed a tracer on the shipment.
NRC, Region III, Chicago, notified Region I of the. incident on July 24, 1979.
They will notify the appropriate states in their Region.
A Region III inspector is onsite at Mallinckrodt for a routing inspection and will review the matter.
There has been no media interest.
Region III is not planning a press release at this time.
The NRC inspector, Region III, Chicago, received. notification of the lost shipment on July 24, 1979, at approximately 3:45 p.m.
The information given reflects the situation as of 8:00 a.m. on July 25, 1979.
