August 3, 1979 PRELIMINARY NOTIFICATION OF EVENT OR UNUSUAL OCCURRENCE--PNO-79-304 This preliminary notification constitutes EARLY notice of events of POSSIBLE safety or public interest sionificance.

The information presented is as initially received without verification or evaluation and is basically all that is known by IE staff on this date.

SUBJECT:

VOID IN SECONDARY SHIELD WALL CONCRETE The Kansas Gas and Electric Co, Quality Assurance Manager notified Region IV by telephone on August 2, 1979, of the discovery of a void in the reactor building secondary shield wall.

During removal of forms for the northwest portion of the concrete secondary shield wall, which was placed on July 24, 1979, a small six inch diameter void was observed in the exterior surface.

The void is located behind an embed for the Loop 1 primary pump tie rod (lateral restraint).

Preliminary evaluation by the licensee indicates that the integrity of the restraint has not been compromised and that the void can L9 repaired using previously approved procedures.

This placement was considered a difficult placement by the licensee based on experience at Callaway.

The licensee had preplanned t'e placement based on the Callaway problems; however, the void still occur,ad.

Similar walls have been placed; however, forms have nat yet been removed.

The licensee reported this matter by telephone to Region IV at approximately 9:15 p.m. on August 2,1979, and provided supplementary information on August 3, 1979.

Because of previous concrete placement problems at this site, the licensee plans to make a news release today regarding the void problem.

A Region IV inspector will be dispatched to the site during the week of August 6, 1979, to further evaluate the effectiveness of the licensee's QA program rela-tive to this event.
