PRELIMINARY NOTIFICATION February 2,1979 PRELIMINARY NOTIFICATION OF EVENT OR UNUSUAL OCCURRENCE--PNO-79-19 This preliminary notification constitutes EARLY notica of event of POSSIBLE safety or public interest significance.
The information presented is as initially received without verification or evaluation and is basically all that is known by IE staff on this date.
FACILITY:
H & H X-Ray Service West Monroe, Louisiana
SUBJECT:
LOST OR STOLEN SEALED RADIATION SOURCE A 41 curie iridium-192 sealed source, contained in a shielded camera, which was stored in a 14 inch pipe that was welded to the steel bed in the rear of an H & H X-Ray Service truck was discovered missing when H & H X-Ray Service personnel arrived at a job site near Yukon, Oklahoma, on February 1, 1979.
The radiography company was not authorized by the NRC to operate in Oklahoma.
The State of Oklahoma has been notified and is currently assisting H & H X-Ray Service in locating the camera and source.
News coverage is expected.
Region IV ( Arlington) received notification of the lost or stolen source and camera by telephone from the State of Oklahoma at 11:50 a.m. on February 1, 1979.
