0, PRELIMINARY NOTIFICATION April 11, 1979 PRELIMINARY NOTIFICATION OF EVENT OR UNUSUAL OCCURRENCE--PN0-79-79 This preliminary notification constitutes EARLY notice of an event of
_POSSIBLE safety or public interest significance.
The information presented is as initially received without verification or evaluation and is basically all that is known by IE staff on this date.

LOST SOURCE OF N0511NAL 0.992 f1ILLICURIES OF 10 DINE-125 DURING SHIPl1ENT The licensee, a manufacturer and distributor of in vivo and in vitro medical diagnostic kits, reported that a shipment of nominal 0.992 millicuries of iodine-125 (liquid) was apparently lost.
The material, a "SEflSOR" fibringen diagnostic kit (for localizing blood clots) was shipped from the licensee's facilities in North Chicago, IL by Purolator, Inc. on flarch 19, 1979.
The material was consigned to Charleston Area Medical Center, Charleston, West Virginia.
On March 27,1979, the consigneee notified Abbott Labs Traffic Department that the kit had not been received.
The licensee's RSO was notified of tha lost material on or about April 4,1979.
The shipment was to have been taken to Palwaukee or fiidway Air Fields, Chicago, Illinois, where Purolator, Inc. was to have it transferred to an aircraft chartered by them (either PDQ or Zantop Airlines) for shipping purposes.
The destination of the chartered aircraft was Columbus, Ohio, where Purolator was to pick up the package and deliver it to Charleston, West Virginia.
The licensee has been informed that Purolator, Inc. personnel have checked possible drop locations to no avail.
They suspect the shipment was delivered to another hospital in the Charleston, West Virginia area and has not been reported.
The licensee is continuing liaison followup with Purolator, Inc. to locate this material.
Preliminary calculations show that about 950 rads to the thyroid and about 4100 rads to the whole body could be received if this material were ingested.

Region II (Atlanta) was. notified by Region III (Chicago) on April 6, 1979.
The States of Illinois, Ohio ana West Virginia have also been notified.
NRC, Region III (Chicago), received notification of this occurrence from the licensee's Radiation Safety Officer by telephone at 10:40 a.m. on April 6,1979.
The information provided reflects the status of this lost material as of 4:30 p.m. on April 10, 1979.
Region III will continue to follow the licensee's progress in locating the material.
