I PRELIMINARY NOTIFICATION OF EVENT OR UNUSUAL OCCURRENCE--PNO-79-50 This preliminary notification constitutes EARLY notice of event of POSSIBLE safety or public interest significance.

The information presented is as initially received without verification or evaluation and is basically all that is known by IE staff on this date.

Subject:

ULTRASONIC TEST INDICATIONS IN REACTOR VESSEL INLET N0ZZLE During the current refueling outage, a preliminary evaluation of in-service inspection (ISI) ultrasonic test (UT) indications from the reactor pressure vessel B primary loop inlet nozzle to pressure vessel weld has shown that they exceed applicable code requirements. The UT signals show an indication of mid-wall slag inclusions in one weld pass with a circumferential dimension of eight inches and a thru wall dimension of one h 'T inch in the nine inch thick, 31 inch I.D. nozzle.

The licensee is meeting with consultants from Westinghouse and Teledyne Engineering, March 9 and 10, concerning the possible need to perform a fracture mechanics analysis.

A meeting with NRR and IE in Bethesda to discuss this matter is tentatively planned for March 19 or 20.

The licensee plans to return to power operation the week of March 26th.

This was the first in-service inspection of this weld.

Previous radiegraphs taken during construction showed some indications of slag within code requirements.

Previous pre-service UT examinations by the vessel fabricator, Babcock and Wilcox and the current ISI contractor, Southwest Research Institute, using different techniques did not disclose tiis situation.

Media interest is expected because of recent problems with other reactor pressure vessel nozzles. The State of New York has been infonned.

A press release is not planned by the licensee or NRC.

NRC, Region I, (Philadelphia) received notification of this occurrence by telephone call from the licensee at 10 a.m., on March 9,1979.
