PRELIMINARY NOTIFICATION AI February 28, 1979 PRELIMINARY NOTIFICATION OF EVENT OR UNUSUAL OCCURRENCE--PNO-79-35 This preliminary notification constitutes EARLY notice of an event of POSSIBLE safety or public interest significance.
The information presented is as initially received without verification or evaluation and is basically all that is known by IE staff on this date.

LOST OR STOLEN 169-YTTERBIUM DTPA SHIPMENT

A package containing 2.6 millicuries of 169-ytterbium DTPA was discovered missing on February 27,1979, at 0800 hours0.00926 days <br />0.222 hours <br />0.00132 weeks <br />3.044e-4 months <br />.
The package was received on February 26,1979, at 10:00 a.m. when it was signed in and surveyed by the licensee.
The package could not be found when it was to be used on February 27, 1979.
After a search, the outer cardboard shipping container was found in the hospital trash.
However, questioning of users and searching storage areas by the licensee were unsuccessful in locating the material.
Direct radiation from the vial is estimated to be 5 R/hr at one centimeter.
Injection of this material for diagnostic purposes results in doses of 38, 3, and 0.2 rads to the spinal column, brain and whole body, respectively.
The Ohio Department of Health was notified on February 28, 1979.
The licensee plans to make a press release on this matter.
NRC, Region III (Chicago) received notification of this occurrence by telephone from the licensee at 0855 a.m. on February 28, 1979.
Region III will follow the licensee's progress by telephone and plans a followup inspection.
