March 7, 1979 PRELIMINARY NOTIFICATION OF EVENT OR UNUSUAL OCCURRENCE--PNO-79-40 V

This preliminary notification constitutes EARLY notice of event of POSSIBLE safety or public interest sinnificance.

The information presented is as initially received without verification or evaluation and is basically all that is known by IE staff on this date.

Facility: Toledo Edison Company, Davis-Besse 1 (DN 50-346)

Subject:

LOW PRESSURE SAFETY INJE; TION SYSTEM IN0PERABLE The licensee reported that while operating at 85% power, a valving error occurred which rendered both decay heat pumps inoperable for a period of 15 minutes.

These pumps provide the low pressure safety injection (LPSI) as part of the Engineered Safety Features Actuation System for the unit.

The valving error involved an inadvertant closure of the motor-operated suction valve for each pump.

The error was discvvered and corrected as a result of a debriefing with the operator following completion of a planned valve lineup change.

These valves are automatically opened during an Engineered Safety Features Actuation.

However, whether or not pump damage would occur under these conditions or whether the required LPSI flow would be achieved is unknown at this time.

The licensee's assessment of this event is in process.

A special inspection is planned and appropriate enforcement action will be taken.

No press release is being planned at this time by RIII.

The licensee does not plan a press release at this time.

The State of Ohio is being notified.

RIII (Chicago) received notification of this event from the licensee on March 6, 1979.
