Subject:

POTENTIAL OVEREXPOSURE FROM RADIOGRAPHIC OPERATION RIII was informed by the licensee of a potential overexposure which occurred at 12:15 a.m. on March 6,1979, from a radiographic operation at the Campbell Unit 3 Fossil Fuel Power Plant in West Olive, Michigan.

A 65 curie iridium-192 source was not fully retracted into the camera when both the radiographer and assistant radiographer entered the area, and the radiation survey performed was inadequate to alert them to the exposed source.

Film badge exposures show doses of 680 mrems and 260 mrems to the upper trunk of the body of the assistant radiographer and radiographer, respectively.

Licensee's calculations show doses of 22 rems to the legs and 5 rems to the lower trunk of the assistant radiographer.

There were no calculations for the radiographer; the doses are thought to be lower than those for the assistant.

An investigation has been initiated by Region III.

NRC, Region III, is considering a press release.

There has been no media interest to date.

The State of Michigan has been informed.

NRC, RIII, received notification of this occurrence by telephone from Townsend and Bottum, Incorporated personnel at 4:00 p.m. on March 7, 1979.
