PRELIMINARY NOTIFIChTION cb January 31, 1979 PRELIMINARY NOTIFICATION OF EVENT CR UNUSUAL OCCURRENCE--PNO-79-17 This preliminary notification constitutes EARLY notice of event of POSSIBLE safety or public interest sianificance.

The information presented is as initially received without verification or evaluation and is basically all that is known by IE staff on this date.

Subject:

LOST OR STOLEN 10 MILLICURIES 125-IODINE SHIPMENT The licensee, a medical research and development institution, reported a missing package of 10 millicuries Sodium Iodide-125 (in solution).

The material was shipped from New England Nuclear (NEN) on January 24, 1979, was carried by Purolator, Inc. and was received at 5:30 p.m. on January 26, 1979 by a licensee representative at the institution.

The material was discovered missing on the afternoon of January 29, 1979.

The licensee has searched his facilities and interviewed personnel, but has been unable to locate the package.

In addition a tracer was placed on the shipment and verified that it was delivered to the institution.

The material does not present a threat to individuals so long as it remains within the shielded container.

Ingestion of the material would deliver a medically significant dose to an individual's thyroid.

The State of Indiana and the local police were notified by RIII on January 31, 1979.

RIII inspectors will be onsite on February 1,1979 to review the licensee's activities related to this matter.

There has been no media interest; however, a press release will be made in an effort to alert anyone who may have the package of the potential danger.

The relea';a will be made by Region III if the licensee does not issue it.

NRC, RIII, (Chicago) received notification of this occurrence from the licensee's Radiation Safety Officer by telephone at 11:30 p.m. on January 31, 1979.

The information given reflects the situation as of 1:00 p.m. on January 31, 1979.
