The information presented is as initially received without verification or evaluation

Subject:
EXPOSED RADIOGRAPHIC SOURCE Region III was informed today by a representative of the Wisconsin Electric Power Company that, during the night of January 15-16, 1979, an incident involving a radiographic device occurred at the company's Pleasant Prairie fossil power plant under construction southwest of Kenosha, Wisconsin.
Radiography was being performed by Magnaflux, a contractor for Wisconsin Electric.
When a radiographer connected the guide tube to a radiographic ca,iera, it did not connect properly. When the source (90 curies Ir-192) was cranked out, the guide tube fell off. The radiographers tried to crank the source back into the camera, but the source snagged in the out position.
The radiographers roped off and posted the area and called their office in Milwaukee.
Two individuals, one the RS0, from the Milwaukee office retrieved the source with long tongs and returned it to a shielded position.
The source in the camera was returned to the Milwaukee office for leak testing.
No overexposures to workers or the public occurred.
The highest exposure was 150 mrems as measured by the pocket dosimeter of a radiographer.
A subcontractor of Wisconsin Electric Power Company working at the site did send 500 employees home today when they showed up for work. This action might result in media interest.
This Preliminary Notification is being submitted because of potential media interest.
No immediate action by Region III is planned.
The matter will be pursued during the next routine inspection.
The State of Wisconsin has been informed.
A press release is not planned by the licensee or NRC.
NRC, RIII, Chicago, received notification of this occurrence by telephone from Wisconsin Electric Power Company at 12:35 p.m., on January 16, 1979 and it was verified at 1:10 p.m. in a telephone call from Magnaflux Corporation.
