April'26, 1979 PRELIMINARY NOTIFICATION OF EVENT OR UNUSUAL OCCURREflCE--PN0-79-94 This preliminary notification constitutes EARLY notice of an event of POSSIBLE safety or public interest significance.

The information presented is as initially received without verification or evaluation and is basically all that is known by IE staff on this date.

Subject:

FIRE AT LICENSEE'S FACILITY The licensee reported that a fire occurred in the basement of its facility at 3:45 p.m. E.S.T where 1.9 microcuries of iodine-125 had been stored.

Entry to the building has not as yet been permitted; however, it is thought that the fire did not reach the 1.9 microcuries of iodine-125.

Fire damage was not extensive and did not reach the upper level of the building where other small quantities of licensed material was stored.

The licensee reported that the fire resulted from a shooting when one of the shots struck a flask of volatile liquid.

One person was killed and the person doing the shooting attempted suicide.

Both were employees of the Medical Center.

The building has been secured pending police investigation.

The State of Indiana was notified by Region III on April 25, 1979.

There was media interest because of the fire and the shooting.

No press release is planned by Region III or the licensee, but questions from the news media will be answered.

NRC, Region III, (Chicago) received notificaticn of this occurrence by telephone on April 25, 1979, at 4:30 p.m.

Region III will maintain contact with the licensee and followup on the next scheduled routine inspection.
