PRELIMINARY NOTIFICATION May 1, 1979 PRELIMINARY NOTIFICATION OF EVENT OR UNUSUAL OCCURRENCE--PNO-79-96 This preliminary notification constitutes EARLY notice of event of POSSIBLE safety or public interest significance.
The information presented is as initially received without verification or evaluation and is basically all that is known by IE staff on this date.

REPORTED RING BADGE READING OF 9,000 REMS The licensee's Radiation Safety Officer (RS0) reported that the routine exposure report for a physician working in X-ray and Nuclear Medicine showed a value of 9,000 rems to the finger badge for the month of February.
The licensee's program is of a size that would make such an expcsure extremely unlikely.
A Region III representative spoke to the physician who stated that his hands showed no signs of erythema (reddening of the skin).
The above dose would have resulted in extensive hand damage by now if it represented an actual dose to the hand.
The licensee has requested the badge vendor to review its result.
Neither the licensee nor Region III plans a press release; however, Region III will respond to media questions. The Indiana State Health Department has been notified.
NRC, Region III (Chicago) received notification of this occurrence by telephone from the licensee's RSO at 3:30 p.m.,
on April 30, 1979.
The information presented herein is current as of 9:15 a.m.,
May 1, 1979.
Region III will perform a special inspection if the vendor confirms that the ring badge actually received 9,000 rems.
