PRELIMINARY NOTIFICATION.
June 4, 1979 PRELIMINARY NOTIFICATION OF EVENT OR UNUSUAL OCCURRENCE--PNO-79-135 This preliminary notification constitutes EARLY notice of an event of POSSIBLE s tfety or public interest significance.
The infermation presented is as initially ~ received without verification cr evaluation and is bas cally all that is known by IE staff on this date.

SUBJECT:
Emergency Feedwater System was Locked Out of Automatic Initiation Mode.

The resident inspector, during a routine inspection of the Unit 1 control room at about 9:00 am on June 2,1979, noted that the Emergency Feedwater System was locked out of the automatic initiation mode.
The plcnt was in a hot shutdown condition.
A main feedwater check valve back leakage surveillance test was in progress.
The surveillance test procedure did not specify locking out the emergency feedwater auto initiation capability.
Lacking a procedural requirement to return the system to normal, there was no assurance that emergency feedwater would be provided automatically if needed. An order was issued June 2, 1979, which required the licensee to return to a cold shutdown condition until the licensee satisfied the following conditions:
(1) the licensee shall evaluate and modify as appropriate its methods for development, review and approval of procedures for all modes of plant operation; (2) the licensee shall evaluate existing procedures to assure that such procedures include all actions necessary for safety; and, (3) the licensee shall take appropriate steps to assure that all plant personnel adhere to approved procedures and not adt unauthorized steps to any procedure."
The plant was verified to be in cold shutdown by the resident inspector on June 3, 1979.
A special NRC inspection (NRR and IE personnel) is in progress.
The licensee has made a press release.
An NRC press release is being prepared.
Region IV, Dallas, initially received notification of the occurrence by tele-phone from the Resident Inspector, at 10:30 a.m. on June 2.
