APPENDIX A Municipality of Anchorage Department of Public Works Construction Division 3500 E. Tudor Road Anchorage, Alaska 99507 License No. 50-15852-01 Notice of Violation Based upon the results of the NRC inspection conducted on June 22,1979, two activities under your license appeared to be in noncompliance with NRC require-ments as identified below:

Contrary to Condition 8 of License No. 50-15852-01, a second moisture density gauge containing 10 mci CS-137 and 60 mci Am-241 was possessed and not authorized on thr "bject license.
The gauge containing the radioactive material is a Nuclear Instruments Corporation l'odel NIC-5.
This is an infraction.
2.
Contrary to Cendition 14A of License No. 50-13852-01, a Campbell Pacific moisture density gauge containing 10 mci Cs-137 and 50 nCi AM-241 was not tested for leakage at a six month interval between the years 1974 and Juae 1979.
This is an infraction.
