Identification of Occurrence:

This R0R concerns the violation of reactor building integrity by opening the outer door of the Emergency Escape Hatch while the inner door was open.

Description of Occurrence:

During power operation, while performing the Semi-Annual Surveillance Test on the Emergency Escap~e Hatch, personnel opened the outer door while the inner door was open, violating containment integrity. Health Physics personnel were monitoring the hatch and there was a slight nega-tive pressure in the hatch.

Analysis of Occurrence: -

Since the reactor building integrity was compromised for only a few seconds and there was a slight negative pressure differential across the hatch.way, there was no hazard to the health and safety of the public.

Personnel were reminded that when the Emergency Escape Hatch Inner Door is being repaired,.all exits should be made through the containment personnel hatch.
