5. Identification of Occurrence:

The failure to perform a Barrel Test on the Emergency Escape hatch and Personnel hatch within the specified time limits.

6. Conditions Prior to Occurrence:

7. Description of Occurrence:

A review indicated that the Barrel Test on the Emergency Escape hatch and Personnel hatch had not been performed within 6 months + 255.since the last such test as required by Technical Specification 4.4.1.2.5 Under Technical Specification 1.3 the hatches were not sealed and Reactor Building Integrity as defined by Technical Specification 1.7 was not maintained as required by Technical Specification 3.6.1.

9. Analysis of Occurrence:

The personnel Hatch. Seals are routinely tested every 3rd day and

'after each entry while maintaining containment integrity. The Escape Hatch Seals are tested after each entry. Subsequent successful Barrel Testing indicated acceptable operability, therefore, there was no hazard to the health and safety of the public.

Surveillance test'ng was successfully completed on 12/17/78 and 12/18/78 for C4 and C2 respectively.
