Appendix A NOTICE OF VIOLATION National Inspection and Consultants, Inc.
License No. 37-18348-01 Based on the inspection cond' acted on April 24, 1980, it appears that certain of your activities were in noncompliance with NRC requirements, as noted below.
The item is an infraction.
10 CFR 71.5 requires licensees, in part, to transport licensed materials in compliance with Department of Transportation regu-lations in 49 CFR Parts 170-189.
49 CFR 172.203(d) requires shipping papers which include ap-propriate entries.
Contrary to the above between April 18 and April 24, 1980, a nominal 100 curie iridium-192 source No. 7655 was shipped by the licensee from Groveton, PA to Perry, OH without shipping papers.
