This refers to your application dated October 8, 1979, requesting approval of your Quality Assurance (QA) program as meeting the QA program requirements of 10 CFR 571.51.

In connection with our review, we need the infomation identified in the erclosure to this letter. Please submit seven copies of your response to the enclosed request for additional information within 30 days following receipt of this letter.

Request for Additional Information 1.

In the second paragraph of Part 1, Part 3, and Part 4 of the QA program, please identify the Corporate Radiation Protection Officer as the individual with the listed responsibilities.

2.

In Part 2, provide a commitment that the Corporate Radiation Protection Officer shall assure that all radioactive material shipping packages are designed and manufactured under a QA Program approved by Nuclear Regulatory Commission for all packages designed or fabricated after the effective date of the QA Program.

(This requirement can be satisfied by receiving a certi-fication to this effect from the manufacturer.)

3.

Add a new section on Document Control with the following commitments:

All documents related to a specific shipping package will be controlled through the use of written procedures. All document changes will be per-formed according to written procedures approved by management.

The Corporate Radiation Protection Officer shall ensure that all QA func-tions are conducted.in accordance with the latest applicable changes to these documents.

4.

Between the first and second paragraphs under, " Handling, Storage, and Shipping," insert the following:

Radiography personnel shall perform the critical handling, storage and shipping operations.

5.

Inspection, Test and Operating Status Inspection, test and operating status of packages for certain special form radioactive material will be indicated and controlled by written procedures.

Status will be indicated by tag, label, marking or log entry. Status of nonconforming parts or packages will be positively maintained by written p rocedures.

6.

Under " Quality Assurance Records," indicate that written procedures and equipment descriptions will be maintained as part of the records.
