l-This is in response to your letter 71-0144 of August 15, 1979 requesting additional information to support AJAX's application for approval of its Quality Assurance Program.
As discussed with Mr. Jack Spraul of your office, the information requested is contained in Chapters 5 and 8 of USS AJAX application for NRC license held in your files under the cover letter AR6/76:ts 9000 Ser 564 of 30 June 1978. A sample copy of USS AJAX Receipt and Transfer Log (NDT Form 9090/1) is enclosed for additional amplification of Chapters 5 and 8.

The Senior P.adiographer will maintain. a cacerd of each receipt and transf er of radioactive material.
2.
Established safety restrictions concerning tha hand 11ag, storage, shioping and receiving of packages for cercEn special form radicactive macerial will be follcwed.
I 3.
Shipments will not be made unless all test, co -ifications, acceptances, and final inspections have been completed.
4.
Work instructions provided for handling, storsge, shipping and re-caiving operations are as follows:
a.
Ordarinu.
(1) The Senior Radiographer will preparu a 20 Form 1348 "CCD Single Line Requisition System Document (manual) and a 1:A*/S.I.:,IDA Form 302 to the O.M Roca for processing. The supply dec. '.ent must be written to ensure that the source is handled on a custody receiph basis. At no time shall the source be lef t unattended during a transfer by a Naval Supply Activity.
The Radiographic Safety Officer shall ensure that the Supply Depa:T.ent is notified that a cust,0?y receipt transfer in required for receiving source.

Transfer.
(1) The Senior Radiographer on board will prepare the radio-ac'ive material for shipment. He shall en.sure that the radioactive r.aterial is shipped on a custody receipt br.is to ensure continual custody during shienent.
e (a) Radioactive sources used for radiography on beard tha 7JAX will,be pachad for shipnant in the proper shipping cantaicar pro-vided by the manufacturer for the particular source.
(b) The source centainer to be used must meet CCT Speci-fication 55 and must be so marked. Briefit/ a Specification 55 container has a lead or uranium shield ccepletely e.cned in a mild steel or other material equally fire resistant. The container should be no ccnstructed

that in the event of fire wi-h resulting melting of the shield r.aterial, a
che shield =aterial cannot flew and lesa G.s effec--iveness.
l (c) The marking en the source con *M er aust be in an un-obatructed area by cetbossing of die stamping on the container, or en a retal plate attached to the container by welding or brazing in letters and figures at least 1/4 inch high. The car %ings. shall be as follows.

These marks shall be understood to cartify the container ccmplias with all specificatica requirements.
