ORGANIZATION Responsibility for th2 Quality Assurance Program for Part 71 requirements rests with Clark Inspection Service, Inc.
This will not cover the fabrication or design of shipping containers for radioactive material.* A Quality Assurance program is established within the limits of our organization structure.

QUALITY ASSURANCE PROGRAM Quality Assurance Program is based on operating procedures with additional emphasis on meeting requirements critical to safety of any shipping package or container.
Radiographers, under direction of Radiation Safety Officer are to be responsible for daily record keeping regarding handling and storage of exposure devices.
The overall program will be under control of the Radiation Safety Officer covering training, documentation and auditing.
The Company uses only cameras and shipping containers manufactured by Ga=ma Industries, who also furnish replacement sources shipped in source changer and shipping containers of their design and certification of which is attached as Appendix A.

DOCLHENT CONTROL All documents related to a specific shipping package will be controlled throught the use of written procedures. All document changes will be performed according to written procedures approved by management.
The Radiation Safety Of ficer shall insure that all. Quality Assurance functions are conducted in accordance with the latest applicable changes to these documents.
4.
HANDLING STORAGE AND SHIPPING Written saf ety procedures concerning the handling, storage and shipping of packages for certain special form radioactive material will be followed.* Shipments will not be made unless all tests, certifications, accaptances and final inspections have been completed. Work instructions will be provided for handling, storage and shipping opetations.
These instructions will be available to all radiographic personnel, which will be drawn from for the purpose of audit.

INSPTCTION, TEST AND OPERATING STATUS Inspection, test and operating status of packages for certain special form radioactive material will be indicated and controlled by written procedures. Status will be indicated by log entry.
Status of non-conforming parts or packages will also be maintained by log entry and disposed as necessary.
Radiography personnel shall perform the regularly required inspections and tests in accordance with written procedures.
The Radiation Safety Officer shall ensure that these functions are performed.
6.
QUALITY ASSURANCE RECORDS Records of package approval (including references and drawings) inspections, tests, operating legs, audit results, personnel training and qualifications and records of shipments will be maintained. Descriptions of equipment and written procedures will also be maintained.
These records will be maintained in accordance with written pro-1 cedures. The records will be identifiable and retrievable. A list of these records, with their storage locations, vill be maintained by the Radiation Safety Officer.

ACDITS Established schedules of sudits of the Quality Assurance Program will be performed using written checklists. Results of audits will be maintained and reported to management. Audit reports will be evaluated and deficient areas corrected. The audits will be dependent on the safety significance of the activity being audited, but each activity will be audited at least once per year.
To assure that all activities are conducted by established guidelines, the General Manager of Clark Inspection Service Inc. shall perform the audit.

AUDITS Continued-The establishment of a record system to log inspection, test and operable status will be in effect to assure proper procedures are carried out.
This record system vill insure that 1.
The tests have been performed.
2.
The equipment is either satisfactory or unsatisfactory.
3.
Action taken if indicated to repair or modify.
4.
Signed off as either usable or removed for disposal by Radiation Safety Officer (QA Part 71).
