APPENDIX A NOTICE OF VIOLATION Northeast Nuclear Energy Co.
Docket No. 050-245 Based on the results of the NRC inspection conducted June 6-8, 1979, it appears that one of your activities was not conducted in full compliance with the conditions of your license as indicated below. This item is a deficiency.
Technical Specification 6.8.1 states " written procedures shall be est;slished, implemented and maintained concerning activities refer-enced below.... C. Surveillance activities of safety-related equipment."
Station Procedure 781.1, Rev. 1, dated January 19, 1978, titled Hydrau-lic Snubber Functional Test, Step 7.2 states, in part, " Select a represen-tative sample of ten snubbers or approximately ten percent of the snubbers, whichever is less, and functionally test them as per MP 739.1."
Maintenance Procedure 739.1, Rev. 3, dated August 2, 1978, titled
" Hydraulic Shock Suppr sso'.' and Sway Arrestor Repair and Testino" Step 5.1 states " Record required information on Maintanance Form 739.11 before removing unit.
Record fluid level for Grinnels as amount reser-voir is full (1/2, 3/4 etc.).
Bergen-Patersons are measured in inchas from point of rod emergence to small drilled hole in one of the wreni:h flats."
Contrary to the above, during the fuactionai e sting of hydraulic snubbers completed May 1, 1979, as part of S.' 781.1 and MP 739.1, snubber fluid levels were not t
- corded for five Grinnel snubbers tested and piston rod emergence was not recorded for the four Bergen-Paterson snubbers tested.
