PRELIMINARY NOTIFICATION September 24, 1979 PRELIMINARY NOTIFICATION 07 EVEh'T OR UNUSUAL CCCURRENCE--PNO-III-79-01 This preliminary notification constitutes EARLY notice of events of POSSIBLE safety or public interest significance.
The information presented is as in,itially received without verification or evaluation and is basically all that is known by IE staff on this date.

LOSS OF 5 CURIE.CS-137 SOURCE IN WELL The licensee reported that a 5 curie Cs-137 well logging source was lost early Sunday morning, September 23, 1979, in a 3900 foot well in Ashtabula, Ohio.~ The licensee could not retrieve the source and plannedl to plug the well with concrete and place a marker over the well.
The ' State of Ohio has been informed.
Neither the licensee n.or Region III -
(Chicago) plan a press release.
News media interest is not expected.
The licensee will submit a 30 day report. Region IV (Dallas) has been notified, since this is a Region IV licensee.
Region III will review the licensee's 30 day report and will inspect the well the next time an inspector is in the area.
Region III received initial notification of this matter by telephone from the licensee at 2:00 p.m.

The Headquarters duty officer was notified.
