STN 50-482/79-17 Appendix A NOTICE OF VIOLATION Based on the results of the NRC inspection conducted on September 17-20, 1979, it appears that certain of your activities were not conducted in full com-pliance with the conditions of your NRC Construction Permit No. CPPR-147 as indicated below:
10 CFR 50, Appendix B, Criterion V states:
" Activities affectIu3 quality shall be prescribed by documented instructions, procedures, or drawings of a type appropriate to the circumstances and shall be accomplished in accordance with these instructions, procedures, or drawings.
Instruc-tions, procedures, or drawings shall include appropriate quantitative or qualitative acceptance criteria for determining that important activi-ties have been satisfactorily accomplished."
Sargent and Lundy Specification A-3854, Paragraph 302.9e, statec:
" Contractor shall assign to the blasting operations and maintain on a full-time basis during the time blasting is in progress, a spec-ialist of mature experience, specialized in the use of explosives for foundation excavation. His qualifications shall be subject to the approval of Purchaser."
Contrary to the above:
The qualification of the blasting specialist assigned by Clarkson (Contractor) to conduct blasting operations, on September 15, 1979, and days fol?: wing, had not been approved by the Daniel Internaticnal Area Civil En/ineer (Purchaser).
This is an intraction.
