Dear Mr. Anderson:

This is in reply to your letter dated October 27, 1979.

Since Three Mile Island, we have been studying the effects of long hours on operator performance. We have not reached a final resolution. However, as a first step, we will establish regulations regarding overtime.

We are also reviewing the means used at nuclear plants for identifying equip-ment out of service. We are considering many systems, including a formal lock and tag procedure.

We agree that more preventive maintenance may have been in order. However, this line has a block valve. The problem was that the personnel did not realize that the relief valve was not seated.
