However, as stated in the telephone conversation with Mr. Davis on March 13, 1977, this report is 2 days late.

Identification of Occurrence:
Title:
River 'Jater Discharge exceeded the inlet river water temperature by 1h F thus exceeding the 12 F limit.
Type: A reportable occurrence as defined by Environmental Specification 5.6.2.3 in that the river water discharge temperature exceeded the inlet river water temperature by greater than the limit of 12 F, thus, exceeding a limiting condition for operation established in Environmental Technical Specification 2.1.b. (1).

Description of Occurrence:
On March h, 1977, at approximately 2000 hours0.0231 days <br />0.556 hours <br />0.00331 weeks <br />7.61e-4 months <br />, the river water discharge temperature exceeded the inlet river water temperature by more than 12CF with a maximum AT reaching 1kOF.
The AT was gradually increasing during the day prior to the incident is a result of:
Increasing and unseasonable high ambient temperatures (peaking at a.
Sh.h F) b.
Nearly 100% relative humidity during the day, rendering the Mechanical Draft Cooling Tower (MDCT) inc?fective to cool the river water discharge.
At 1300 hours0.015 days <br />0.361 hours <br />0.00215 weeks <br />4.9465e-4 months <br /> the fans were secured to reduce the heating effect of the towers. At approximately 2000 hours0.0231 days <br />0.556 hours <br />0.00331 weeks <br />7.61e-4 months <br /> the AT chart recorder in the Control Room appeared to have leveled off at 11.6 F, but had, in fact, reached the techanical stop. The operator continued to atte=pt to reduce the AT by increasing total River Water flow but was unsuccessful until the standby Decay Heat River Water pump was started. After 15 minutes of operation the DERW pump was stopped in order to decrease the rate of AT change. The AT was belcw 12cy, 7.
Apparent Cause of Occurrence:
This event has been determined to result from a ec=bination of the following:
a.
The existing procedures lack the specific corrective action to take when high ambient temperature and relative humidity ec=bined with low river water temperature cause an increasing AT.
b.
A personnel error was e contributing factor in that, ence the AT chart reccrder was against the mechanical stcp, the back-up AT chart in the MDCT Pu=p House was not i==ediately inspected to determine the AT.
c.
The MDCT was ineffective due to the abnormal 1=bient conditions and the low river water temperature.

Analysis of Cccurrence:
It has been determined that this occurrence did not adversely affect the environment in that the 2T excursicn to a maximum of lho? lasted only a short period of time.

Corrective Acticn:
In ad !ition to the i= mediate action described above:
e.
All licensed operators will be briefed on this AT excursion, the requirements to use back-up instrumentation to assure compliance with Technical Specifications, and the sequence of corrective measures to be taken in the event of a similar AT excursion.
b.
Section 37.3.h.2 of the MDCT operating Procedure vill be revised to provide expended guidance to assure the AT Technical Specificaticns limits are not exceeded.
The Plant Operation Review Cc=mittee and Unit Superintendent have reviewed and approved the above corrective action and have taken steps to assure its completion.
