On September 10, 1979, witn the reactor in a shutdown condition, engineering personnel observed that cold reheat snubber CRS-262 was inoperable.

Since the reactor had been operated at power during the surveillance interval, this constitutes operation in a degraded mode of LCO 4.3.10, and is reportable per Fort St. Vrain Technical Specification AC 7.5.2(b)2.

EVENT

DESCRIPTION

On September 10, 1979, with the reactor in a shutdown condition, engineering personnel o'., served that the pin at the piston rod eye of cold rcheat snubber CRS-262 was missing. See details of Figure 1.

Pecause this pin was missing, the snubber was not properly attac' ed to the pipe, and was therefore inoperable. This snubber had been verified operable per approved surveillance procedure during the refueling shutdown.

CAUSE

DESCRIPTION:

Prior to startup following the first refueling, this snubber was operable.

There appears to be no cause for this missing pin traceable to plant opera-tion.

CORRECTIVE ACTION:

A new pin was fabricated and installed and the snubber was returned to service.

The present surveillance interval is 12 months + 25%. No change is required as a result of this occurrence.

No further corrective action is anticipated or required.
