PRELIMINARY NOTIFICATION September 19, 1979 PRELIMINARY NOTIFICATION OF EVENT OR UNUSUAL OCCURRENCE--PNO-79-429 This preliminary notification constitutes EARLY notice of an event of POSSIBLE safety or public interest significance.
The information presented is as initially' received without verification or evaluation and is basically all that is kncwn by IE staff on this date.

SUBJECT:
APPARENT OVEREXPOSURE OF ASSISTANT RADIOGRAPHER A radiographer and an assistant radiographer noted that their pocket dosimeters were off scale after performing radiography at a temporary job site in Dale, Indiana on September 17, 1979.
The assistant radiographer was not wearing his film badge on September 17,.1979.
g apparent cause of the off-scale dosimeters was failure to return the 53 Curie Ir source to its shielded oosition follow-ing a radiography procedure.
The radiographer's film badge was submitted on September 18, 1979 for processing.
Reenactment of the incident by the licensee indicated whole body exposures of 500 mR to the radiographer and 5 R to the assistant radiographer.
Region IV (Arlington, TX) received notification of the incident by telephone from the licensee at 9:30 on September 18, 1979.
Region IV, with assistance from RIII, will investigate the incident.
