Orqanizatun The final responsibility for the Quality Assurance (QA) Program for Part 71 Recuirements rests with WI. INDUSTRIAL TESTING. INC.

(Name of Licensee)

Design and Fabrication shall not be conducted under this 'jA Program.

The QA Procram is implemented as shown on the attached organization chart.

The Radiation Safety Officer is resJonsible for overall administration of the program, training and certification, document control, and auditing.

The Radiographers are responsible for handlina, storing, shipping, inspection, test and operating status and record keeoing, 2.

nuality Assurance Procram The management of WI. INDUSTRIAL TESTING, INC.

(Licensee Name) astablisnes and implemnts this QA Program. Training, prior to engagement, fer all QA functions is requ' ed according to written procedures. QA Denaram revisions will be made accordina to written nrocedures with management acproval. The CA Procram will ensure that all defined QA procedures, engineering procedures, and specific provisions of the package desian approval are satisfied. The QA Program will emphasize control of the characteristics of the cackage which are critical to safety.

The Radiation Safety Officer shall assure that all radioacti te material shippino 9,ickages are desioned and manuf actured under a OA Program approved by 'suclear Reguistory Comission for ill packanes desi?ned or fabric 3ted after July 1,197R. This requinment can be satisfied by receivinq a certification to this effect from the nanufacturer.

Document Control All documents related to a specific shipping nackaae will be controlled througn the use of written procedures. All document chances will be perfomed accordino to written procedures approved bv management.

The Radiation Safety Officer shall insure that all 'M fections are conducted in accordance with the latest aonlicable changes to these documents.

Handling Storme and Shioci_nj Writ *en safety procedures concerning the 5ardlinq Storace and shipping of packaces for certain special form radioactive material will be followed. Shipments will not be ade unless all tests, certifications, acceptances, and final inspections have been completed. Work instruct-ions will be provided for haadling, storace, and shiocing operations.

Radiography personnel shall perform the critical handling, storage and shipping operations.

Inspection. Test and Ooerating Stctus Inscettion, test and ocerating status of packaces for certain scecial form radioactive material will be indicated and controlled by written crocedures. States will be indicated 5y tag, label. markinq or loa entry. Status Y nonconformino parts ne packaces will be positively maintained by written procedures.

Records of package approvals (including e?ferences and drawings),

procurev at, inspections, tests, operating logs, audit results, personnel training ind cualifications and records of shipments will be Paintained.

Descriptions cf equip ent and written proc &res will also be maintained.

These records will be maintained in accordance with written procedures.

The records will be identifiable and retrievable. A list of these reccrds, with their storaqe locatinrs, will be maintained by the Radiation Safety Officer.

7.

Audits Established schedules of audits of the OA Program will be performad using written check lists. Results of audits will be maintained and reported to management. Audit reports will be evaluated and deficient areas corrected.

The audits will be dependent on the sifety siqnificance of the activity being audited, but each activity will be iudited at least once per year.

Audit reports will be maintained as part of the cuality assurance records.
