h NOTICE OF VIOLATICN University of Oklahoma Docket No. 30-02855 Health Science Center License No. 25-03176-01 As a result of the inspection conducted on February 3 L 4, 1981, and in accordance with the attached Interim Enforcement Policy, 45 FR 66754 (October 7, 1980),
the following violation was identified:
License Condition 16 requires that licensed material be used in accordance with statements, representations and procedures contained in application dated December 20, 1973 and letters dated June 14, 1974, Decemoer 16, 1972, May 23, 1975, June 16, 1975, June 2S, 1977 and October 27, 1977, which states in part that records of waste inventory for disposal shall be maintained by each user.
Contrary to this requirement, the inventory of waste in Room 735 located in the Biomedical Sciences Building was not maintained.
This is a Severity Level V violation.

Pursuant to the provisions of 10 CFR 2.201, University of Oklahoma, Health Science Celter is hereby required to submit to this office within twenty-five (25) days of the date of this Notice, a written statement or explanation in reply, including:
(1) the corrective steps which have been taken and the results achieved; (2) the corrective steps which will be taken to avoid further violations; and (3) the date when' full compliance will be achieved.
