Appendix A NOTICE OF VIOLATION Ingham Medical Center License No. 21-12826-01 As a result of the inspection conducted on February 24, 1981, and in accordance with the Irterim Enforcement Policy, 45 1'R 66754 (October 7, 1980), the following violation was identified:
1.
10 CFR 35.14(e)(1)(i) requires that sealed calibration or reference sources possesse-1 pursuant to 10 CFR 35.14(d) be tested for leakage and/or contamina tion at intervals not to exceed six months.
Contrary to this requirement, as of the day of the inspection, you have not leak tested your 1 mci Cs-137 calibration source since November 24, 197o.
This is a Severity Level V violation (Supplement VII).

Condition No. 16 of your license states the dose calibrator shall be calibrated in accordance with the procedures described in Appendix D, Section 2, NUREG-0338, Revision 1, dated November 1, 1977.
Appendix D, Section 2 requires a quarterly linearity calibration of the dose calibrator.
Contrary to the above, on the day of inspection, records showed the linearity calibration was last performed on May 5, 1980.
This is a Severity Level V violation (Supplement VII).
Pursuant to the provisions of 10 CFR 2.201, you are required to submit to this office within twenty-five days of the date of this Notice a written statement or explanation in reply, including for each item of noncompliance:
(1) corrective action taken and the results achieved; (2) corrective action to be taken to avoid further noncompliance; and (3) the date when full com-pliance will be achieved.
Under the authority of Section 182 of the Atomic Energy Act of 1954, as amended, this response shall be submitted under oath or affirmation.
