SEP 0 71979 FCTC:EB 71-5768 Those On Attached List Certificate of Compliance 5768 for the Model No. 8B-250-2 expires on November 30, 1979.
On June 6, notice of the expiration of this certificate was sent to the Combustion Engineering Company, the primary user, requesting a consolidated application for renewal.
In a September 5,1979, telephone conversation with Ellen Brown,. George Bakevich of Combustion Engineering indicated that they would not request renewal.
If you desire to continue use of this package, a consolidated application for renewal should be submitted by one of the users.
Other users should request continuation as a user of the certificate in accordance with Paragraph 71.12(b) of 10 CFR Part 71.
For renewal, please provide the consolidated application thirty (30) days before the expiration date of the certificate.
Eight copies of your application should be provided.
In addition, the renewal fee prescribed in 10 CFR sl70.31 should be included with the consolidated application and request for renewal of the certificate.
This notice is sent for your convenience and it should not be interpreted that similar notices will be sent in the future.
