Appendix A NOTICE CF VIOLATION Wisconsin Industrial Testing License No. 48-17480-01 Company Based on the inspection conducted on July 9, 1979, it appears that one of your activities was in noncompliance with NRC requiremente, as noted below. This item is a deficiency.
Condition 18 of your license requires that licensed material be possessed and used in accordance with statements, representations, and procedures contained in application dated July 25, 1979. Section BC-1 of this application states that survey instruments in use will be calibrated by one of two specified firms.
Contrary to this requirement, as of the date of this inspection, your su vey instruments were being calibrated by a firm other than one of the two specified in your license application.
