1 Public Service Comp,ty of Colorado Fort St. Vrain Nuclear Generating Station Docket No. 50-267/79-13 NOTICE OF VIOLATION l Based on the results of the NRC inspection conducted July 10-13, 1979, it_ appears that certain of your activities were not conducted in full compliance with N2C requirements as indicated below:

Technical Specification AC 7.4.a. requires in part that, " Written procedures shall be established, knplemented and raintained.

Administrative Procedure.(ADS! No. 28 for Data Logs, Data Sheets, and Charts, Section 3.2 states in part that, "When the reaponsible person changes a chart or recorders, and once per shift, be sure that the recorder is inking.

cate that the pen was or had not been inking. Again en July-13, 1979,.

at 8:10 a.m. this recorder was observed not to be inking and had nbc been inking since 2:00 p.m. on July 12, 1979. Although 1he chart was dated at 12:00 p.m. on July 12, 1979, no PTR was written to indicate that the instrument was not inhing properly. Moreover, on July 11,1979, at 8:30 a.m. the PCRV surface temperature recorder of T 4637 was. observed not to be inking. The chart'had been date stamped on July 10, 1979, at 12:00 p.m. but the chart had not been inking sinca 2:00 p.m. on July 10, 1979.

Again, contrary to the requirements stated above, no PTR had been

written, b.

ADM Procedure No. 28, Section 2.3 r:atai taat, "The person resp'ansibfe for obtaining the data shall take tae data at thw specified intervals, if defined, tc. the data log and/or data sheet, noiess specifically exempted by 1 Operations Order (per ADM 24). AA1 log sheets are tn be completed (dates,' times, names, etc.) by the operator unless spet-ifically exenpt'by nn Operations Order."

The Operations Order for System 14 dated R,rch 26, 19'/ ), states that

" Readings of tamperatures on all fuel storaga wells shall be t2L-n ac such times as ind.icated by the attached log anytime there is new t'uel or irradiated fue.1 in the storage wells.- Log:.he namreraturr. and initial in appropriate column."

2 Contrary to the above, no temperature readings for the fuel storage wells were taken at 1400 on March 31, 1979; at 1600, 1800, 2000 and 2200 on June 10, 1979; at 0600, 0800, 1000, 1200 and 1400 on June 14, 1979; and at 1400 on June 26, 1979, nor were there any reasons indicated for the omission of th<2 data taking. Moreover, the data sheets did not appear to have been reviewed as required by Section 2.1 and 2.8 of ADM No. 28.

This is an infraction.

Technical Specification AC 7.4a reouires in part that, " Written procedures shall be established, implemented and maintained.

Administrative Procedure (ADM) No. 12 for Maintenance and Repair Procedures, Section 5.1.3(10)f states that work / test instructions include the following instructions to personnel performing work and/or tests:

" Sign off where applicable all records and data sheets.

Indicate not applicable (N/A) on all signoff blanks associated with steps that were not required to complete the necessary repairs."

Contrary to the above, Step 9.6.2 and all of page two (2) of thintenance Procedure (MP) No. 4 utilized for repair of Valve V-11752 were not signed off, nor were they indicated as N/A.

This is an infraction.
