Appendix A NOTICE OF VIOLATION Midwest Inspection Services, License No. 48-16296-01 Ltd.
Based on the inspection conducted on June 6, 1979, it appears _that certain of your activities were in noncompliance with NRC requirements, as noted below. Items 1 and 2 are infractions.
1.
10 CFR 20,207(b) states licensed materials in an unrestricted area and not in storage shall be tended under the constant surveillance and immediate control of the licensee.
Contrary to this requirement, on June 6,1979 a Gamma Century Exposure Device, Serial No. 20.43 containing a 10 curie iridium-192 sealed source was lying on the ground in an unrestricted area while the radiographer was asleep approximately 100 feet away in the radiography truck.
2.
10 CFR 20.203(c)(1) states each high radiation area shall be posted with a sign or signs bearing the radiation caution symbol and the words: Caution - High Radiation Area.
Contrary to this requirement, two radiographers whc were interviewed during a field inspection on June 6,1979 stated they do tot use Caution - High Radiation Area signs when making radiographJe exposures at field locations.
