Appendix A NOTICE OF VIOLATIuN Dairyland Power Cooperative Docket No. 50-409 Based on the inspection conducted on June 4-7, 1979, it appears that certain of your activities were in noncompliance with NRC requirements, as noted below.
This item is a deficiency.
10 CFR 71.62 requires that certain records be maintained for shipments of greater than type A quantities of radioactive material.
The required records include the results of the following determinations required by 10 CFR 71.54:
(1) the packaging has not been significantly damaged; (2) the closure of the package and any sealing gaskets are present and are free from defects; (3) the package has been loaded and closed in accordance with written procedures.
Contrary to the above, records were not available to substantiate that determinations of packaging damage, package closure and sealing device status, and package loading and closing in accordance with written pro-cedures, had been made for three type B shipments over the period from June 1978 through the dates of this inspection.
