i Appendix A NOTICE OF VIOLATION Twin Ports Testing, Inc.
License No. 22-15932-01 Based on the inspection conducted on July 12 and 13, 1979, it appears that one of your activities was in noncompliance with NRC requirements, as noted below. This item is an infraction.
License Condition 16 requires that licensed material be possessed and used in accordance with statements, representations, and procedures contained in applications dated December 18, 1973; May 21, 1974 and January 26, 1976; and letter dated January 23, 1976.
Item 3 of applicatior iated January 26, 1976 states that sources will be stored at Room 175, Meierhoff Building, 341 Lake Avenue, South, Duluth, Minnesota 55811.
Contrary to the above on the dates of this inspection, your Ir-192 sources were stored at 316 Garfield Street, Duluth, Minnesota, a location not authorized by your license.
