Limitorque Corporation Docket No. 99900100/79-01 NOTICE OF DEVIATION Based on the results of an.NRC inspection conducted on March 26-29, 1979, it appears that certain of your activities were not conducted in full accordance with NRC requirements as indicated below:
A.
Criterion V of Appendix B to 10 CFR 50 requires that activities affecting quality shall be prescribed by documented instructions, appropriate to the circumstances.
The QC Manual, paragraph 5.C.1 identifies welding as a special process.
Contrary to the above, standard operating procedure QCP-12, Revision 1, subprocedure 12.F, " Welding," used in the welding of " Floor Stands (supports for the valve operators)" was not appropriate in that the procedure did not provide welding parameters necessary to assure that welding is controlled _.
B.
Criterion XVII of Appendix B to 10 CFR 50 requires that sufficient records shall be maintained to furnish evidence of activities affecting quality. The records shall include closely-related data such as qualifications of personnel.
The QC Manual, paragraph 5.C.5, states in part, "A Welding Qualifica-tion Record will be kept of those men designated ' Welders' by Limitorque.... " Paragraph 5.C.4.a states in part, "It is the responsibility of the Quality Control Department to mcintain the Welding Qualification Records."
Contrary to the above:
1.
A Welding Qualification Record was not kept for the person performing the welding on operator floor stands.
2.
The Quality Control Department was not maintaining Welding Qualification Records.
