b APPENDIX A NOTICE OF VIOLATION Green Construction Company License No. 47-19164-01 Based on the NRC inspection October 16, 1980, certain of your activities were apparently not ccnducted in full compliance with NRC requirementr, as indicated below. These items have been categorized as described in correspondence to you dated December 31, 1974.
As required by Condition 12 of your license, l' censed material shall be used by, or under the supervision of, Michael J. Minckler.
Contrary to the above, i t. was determined that licensed material has not been used by or under the supervision of Michael J. Minckler as he left the employ of Green Construction Company in June, 1980. Licensed material has been used by D. Neely since June,1980.
