Appendix A NOTICE OF VIOLATION Detroit Edison Company Docket No. 50-341 Based on the inspection conducted on May 8-11, 1979, it appears that certain of your activities were in noncompliance with NRC requirements, as noted below.

Items 1, 2 and 3 are infractions.

1.

10 CFR 50, Appendix 3, Criterion V, and Edison Quality Assurance (QA) Manual, Section 9, Paragraph 9.15 states in part:

" Activities affecting quality shall be prescribed by documented instructions, procedures and drawings

. and shall be accomplished in accordance with these instractions, procedures or drawings.

Contrary to the above, the following conditions were identified:

a.

Wismer & Becker purchasing, engineering, construction and quality personnel did not receive the minimum training as prescribed in their Quality Assurance and Control Manual,Section IV, Paragraphs 4.1.1, 4.1.2 and 4.2.1.

Additionally, contractor responsibilities were net adequately discharged as outlined in Paragraphs 3.1, 3.2, 3.3 and 3.4.

b.

Three Level II welding inspectors did not meet the minimum experience r guirements of Wismer & Becker procedure QA-TM-1 Paragr aph 5.5.1.1.2 which states that the inspector shall be a high school graduate and have four years inspec-tion / test experience.

In addition, another inspector's record of experience was not in the file.

Penetrameter No. 12 did not have lead numbers to identify the penetrameter as required by Vismer & Becker procedure, WB-Q-110, Section 6.4.2, which states that lead identifi-cation numbers shall be permanently attached.

d.

Wismer & Becker procedure WB-Q-110, did not contain measures describing the proper use and positioning of penetrameters.

ASME Code,Section V, Article 22, SE-142 describes precise manufacturing requirements for penetrameters in order to determine radiographic quality. Penetrameters had been bent to fit the radius of small pipe, thus, distorting the penetrameter's dimensional tolerances. The procedures should describe proper penetrameter positioning, i.e.

that a shooting sketch for each technique shall be utilized.

Separrte shooting sketches for field welds, FW-P-44-3558-1 WF1 and B21-4416-3W F1 as well as sketches for other various standard and special pipe sizes, were not prepared.

10 CFR 50, Appendix B, Criterion IX, and Edison QA Manual QA9 10, states in part:

" Measures shall be established to assure that special processes including welding, heat treating and nondestructive testing, are:

controlled; and accomplished by qualified personnel.

Contrary to the.=bove, the following conditions were identified:

a.

Certifications of four Daniel International Company Level II NDE inspectors failed to meet the requirements of Daniel procedure 7.1, Section A.4.3, which requires demonstrated proficiency in selecting, and performing examinations and evaluating test results obtained.

Examination to certify inspectors only covered evaluation of test results, yet, the certification was not restri:ted to only this area.

b.

A Wismer & Becker examinee who failed an examination was retested after a fifteen day waiting period and was certified without receiving additional training as required by Wismer & Becker procedure QA-TM-2, Section 6.3.3, which states in part "

a retest is permissible after a fifteen day waiting period provided additional training is furnished...."

c.

Wismer & Becker Level III NDE (site) personnel were not certified as required by Wismer & Becker QA/QC ManualsSection IX, Paragraph 4.6.4.3 which states in part

. The Corporate Quality Assurance and Control Manager shall certify the site Level III."

Site personnel were certified by someone other than the corporate QA/QC manager.

d.

Wismer & Becker NDE personnel did not make the minimum test acore required by WB procedure QA-TM-2, Section 6.3.2 which requires a minimum test score of 70.

determine qualifications, however, the examination did not meet the requirements of WB proce. dure QA-TM-2, Section 6, which requires both a specific and a general examination.

General type questions for SNT-TC-1A (Liquid Penetrant) were given on both the specific and general examination.

f.

Nuclear Energy Services (KES) personnel records failed to indicate the visual acuity (color contrast) test results.

NES procedure 99-CNTP-001, Paragraph 6.2, requires docu-mented results of such visual acuity tests.

10 CFR 50, Appendix B, Criterion XII, and Edison QA Manual, QAP 13, which states: " Measures shall be established to assure that tools, gauges, instruments and other measuring and testing devices used in activities affecting quality are properly controlled, calibrated and adjusted at specific periods to maintain accuracy within necessary limits."

Contrary to the above, the following conditions were identified:

a.

Daniel International failed to calibrate densitometers with calibrated step wedge film as required by Section 3.6 of Daniel procedure QCP-V1-01, Rev.

1, " Quality Control Procedure for Interpretation of RT Film."

In additional Daniel procedure AP-VII-04, " Calibration and Control of Measuring and Test Equipment," Rev. 4 did not provide measures for assuring that step wedge film strips were periodically certified /recertified.

b.

Wismer & Becker failed to calibrate step wedge film used to calibrate their densitometers.

Procedure WB-Q-110 requires calibration of densitometers by checking with a calibrated step wedge film. Again the calibration and measurement cf test equipment procedure failed to require the calibration of film strips used to calibrate densitometers.

Nuclear Energy Services performs radiography tests to the Wismer & Becker procedure WB-Q-110.

One film strip they were using had never been calibrated.
