This refers to the telephone contacts on May 14 and 15, 1979, between you and 'd. H. Schultz of this of fice regarding the failure of Superior Indust. rid X-ray Company personnel to maintain constant surveillance over a radiographic expor ;e device containing a iridium 192 sealed source which was in an unrestricted area.

Specitically, one of your radiography trucks was at the Fermi Laboratory in Warrenville, Illinois, on May 14, 1979, at which time a radiographic exposure device was placed on the tail gate of the truck awaiting a survey by Fermi health physics personnel. When the health physicist arrived to nake ti survey he noted the exposure devica was not under the surveillance of radiographer who was in the cab of the truck. The health physicist then took possession of the exposure device and after several hours returned it to the radiographer.

It should be noted that failure to maintain constant surveillance and immediate control of licensed materials in an unrestricted area is not in cccordance with the requirements of 10 CFR 20.207.

This matter will be reviewed during the next scheduled inspection.
