License fio. 11-12896-02 flotice of Violation Based on the results of the f1RC inspection conducted on March 27-28, 1979, certain activities appear to be in noncompliance with flRC regulations and the conditions of your flRC License No. 11-12896-02 as indicated below:

1.

10 CFR 34.24 requires in part that radiation survey instruments be calibrated at intervais not to exceed three months.

Contrary to this requirement, Gamma Industries Model 350, Serial 3002, survey meter last calibrated on September 23, 1978, was used in conjunction with radiography at the Laramie River Power Station, Wheatland, Wyoming, on January 15, 1979.

This is an infraction.

2.

License Condition 16 requires in part that licensed material be used in accordance with statements, representations, and procedures con-tained in the letter dated December 12, 1974.

Contrary to this requirement:

a.

Inspection and maintenance of radiographic exposure devices, required to be conducted at intervals not to exceed three months in accor-dance with the Radiation Training and Safety Manual,Section II, Part B, Iten 9.D.1, was not performed on the device at Billings, Montana, during the period September 23, 1978, to January 20, 1979.

b.

Inspections of personnel and equipment, required to be conducted quarterly in accordance with the Radiation Training and Safety Manual,Section II, Part C, Item 6, were not performed on personnel and equipment at the Billings, Montana, office during the period of September 3,1978, to January 10, 1979.

This is an infraction.

3.

10 CFR 34.26 requires in part that each licensee conduct a quarterly physical inventory to account for all sealed sources received and possessed under his license.

Contrary to this requirement, a physical inventory of sealed sources possessed 2.t the office in Billings, Montana, was not conducted during the period flovember 8,1976 to April 7,1977.

This is a deficiency.

License Condition 13. A(1) requires that sealed sources shall be tested for leakage and/or contamination at intervals not to exceed six months.

Contrary to this requirement, the sealed source used in Troxler gauge Model 3401, Setial 4187, in use at Great Falls, Montana, was not tested for leakage and/or centamination during the period April 22, 1977, to Decenber 12, 1978.

This is an infraction.
