Provide an organization chart which shows the reporting relationship of personnel (by title) involved in the QA program.
2.
Identify what personnel (by title) perform the critical handling, storage, and shipping operations.
3.
Identify whet personnel (by title) perform the regulatory required inspections and tests in accordance with written procedures and who (by title) ensures that these functions are performed.
4.
Under " Audits," provide a statement that each activity will be audited at least once each year by an auditor (s) who has no responsibility in the activity being audited.
